The submission of this Electronic Label Rental for Credentialed Dietetics Practitioners Form is subject to the approval of the Commission on Dietetic Registration (CDR) in its discretion. Upon review of the information submitted, you will receive an e-mail communication advising whether your request has been approved. Please allow five days for approval notification. Please note that CDR reserves the right to request additional information upon review of information submitted. I agree to rent mailing list/labels from the Commission on Dietetic Registration subject to the following conditions: 1. List/labels will be used only one-time direct mail purpose for which it was approved. 2. List/labels will not be duplicated, resold or integrated into a permanent database. 3. Payment for the list/labels use must be received within 30 days of the date of the invoice. Having read the above, I agree to assume full responsibility for any failure to comply with these terms. Non-compliance will disqualify the undersigned from receiving future lists/labels from the Commission on Dietetic Registration. I have read and agree to the above terms and conditions.
