Rental Regulations

• The label rental is contingent upon approval of the user’s purpose, which must be consistent with CDR purpose. Product statements on properties, performance, nutrient values, beneficial results, etc., must be verified by adequate data available in peer-reviewed-evidenced-based literature.

• Each rental request must be accompanied by a completed rental agreement (21.1KB, available in PDF format. For more information on PDF format please click here) and a copy of the entire intended mailing or a final draft copy of the mailing as presented to the printer if the deadline prohibits submission of the actual mailing.

• Approval of specific requests will be based on review of the mandatory mailing sample and the application agreement and credit approval.

• Labels are rented for one-time use only. Labels must not be duplicated, transferred, or sold to a third party, or used for any purpose other than the expressed purpose. Violation of these specifics will be grounds for denial of any future rental request.

Option 1: Credentialed Status

Ordering labels by this option offers users the advantage of reaching more than 74,000 Registered Dietitians (RD); more than 4,000 Dietetic Technicians, Registered (DTR); 354 Board Certified Specialists in Renal Nutrition (CSR); 365 Board Certified Specialists in Pediatric Nutrition (CSP); 59 Board Certified Specialist in Sports Dietetics (CSS); 34 Board Certified Specialists in Gerontological Nutrition (CSG).

Option 2: State of Residence

Ordering labels by this option helps users pinpoint credentialed dietetics practitioners in geographic areas where specific services and products are marketed.

Option 3: City of Residence

Ordering labels by this option helps users identify credentialed dietetics practitioners by city.

Option 4: Zip Code

Ordering labels by this option helps users identify credentialed dietetics practitioners by zip code.

Option 5: Recertification Period

Option 6: Certificate of Training in Adult Weight Management Recipients

Ordering labels by this option will provide users a list of those individuals who have successfully completed the Certificate of Training in Adult Weight Management (over 1000). This is a subgroup of the recipients as it reflects only those who have given CDR permission to release their contact information.

Option 7: Certificate of Training in Childhood and Adolescent Weight Management Recipients

Ordering labels by this option will provide users a list of those individuals who have successfully completed the Certificate of Training in Childhood and Adolescent Weight Management (over 500). This is a subgroup of the recipients as it reflects only those who have given CDR permission to release their contact information.

Formats and Pricing:

• CD-ROM (Excel File): $130.00 per thousand names plus $25.00 non-refundable disk charge

• Electronic (Excel File): $130.00 per thousand names plus $50.00 non-refundable transmission charge

Minimum order: 1,000 names or $130.00.

Additional Selections

• For Pressure-Sensitive Labels: Order changes after printing is completed will be charged an additional $50.00

• Shipping charges will be added to the invoice.
