The following services are available at no cost to RDs and DTRs:

Change of Name Policy
For the protection of your identity, name changes cannot be made on the website, but must be submitted with legal documentation via mail to the Academy or CDR.  Documentation must include the original legal document such as marriage license/certificate, divorce decree or court order showing the name change.  If you do not wish to submit the original document, a notarized copy will be accepted.  We are unable to return original documents or the notarized submittal. Please be sure to place your previous name and your Academy/CDR identification number on your correspondence and include how you want your new name to be entered into the database. Name changes can be sent to:  Academy of Nutrition and Dietetics or Commission on Dietetic Registration, 120 South Riverside Plaza, Suite 2000, Chicago, Illinois  60606.  We are unable to accept faxed or scanned copies attached to emails.  Should you have any questions regarding your name change, please do not hesitate to contact us.

Change of Address/Email Policy
To protect the security of your record and maintain integrity of the Academy/CDR database, we are unable to change mailing address and email address based on a verbal requests. We encourage you to log into your profile by clicking here to update your mailing address and/or email address. You may also send a written request, along with your Academy/CDR identification number to cdr@eatright.org or mail to: Academy of Nutrition and Dietetics or Commission on Dietetic Registration, 120 South Riverside Plaza, Suite 2000, Chicago, Illinois  60606. Faxes will not be accepted.

Monitoring of CDR Credential Use/Misuse

Monitoring Compliance with the Academy/CDR Code of Ethics for the Profession of Dietetics
To report violations of the Academy/CDR Code of Ethics for the Profession of Dietetics, send an e-mail to ethics@eatright.org.

To locate CDR-approved CPE activities by location, date, and/or topic, utilize our Online CPE Database or submit your request along with your name and fax number or address by e-mail to:

Submission of Professional Development Portfolio Learning Plan and Logging of CPEUs on Professional Development Portfolio Activity Log.
To submit PDP learning plans and log CPEUs on the PDP activity logs, visit the Academy/CDR on-line business center. To access your record, you will need your registration number and web password.

On-Line Verification of Registered Status
To verify registered status on-line, go to the CDR home page and click on the On-line Verification Search icon. To obtain verification, the registration identification number and state of residence must be provided.

Written Verification of Registered/Certified Status
To request written verification of your registered/certified status, submit your request with your name, registration number, and address by e-mail at cdr@eatright.org or phone at 312-899-0040 Ext. 5500.

CPE Request for Prior Approved Programs
