How to upload name change legal documentation?
Please click here to log in to your MyCDR account using your CDR/Academy username and password. Once logged in, click on the “Profile” link on the left side of the blue bar. Once you have clicked the “Profile” link, you will see the “Request Name Change” link. Click the “Request Name Change” link and follow the instructions to upload your legal documentation such as marriage license/certificate, divorce decree or court order reflecting the name change. You will receive an email notification once your request has been processed. Please allow 72 hours for processing. If you do not wish to use this feature to upload your documentation, please click here to view the process to mail your documentation to CDR/Academy.
