Registration ID Cards

Upon paying their registration maintenance fee, all CDR credentialed practitioners will receive a registration ID card in the mail. This official card is mailed 2 weeks after payment is received and sent to the primary address on the practitioner’s CDR profile.

Newly credentialed practitioners will also receive a complimentary wall certificate. In subsequent years, practitioners may order a wall certificate here:  

What if I lose (or do not receive) my card?


If you lose your registration ID card, a new , official card can be requested by emailing CDR@eatright.org. Please include your registration number in this email. If you need a temporary copy of your card, you can request one be emailed to you using the below procedure.

To request a temporary card: 
  • Sign in to your MyCDR page.
  • On the light blue menu bar, select the CDR ID Card option.
  • After confirming your name, email and credential, press Submit. Within an hour, you will receive an email containing a pdf copy of your card.
