Change of Name/Address Policy

To protect the security of your CDR record and maintain the integrity of the CDR/Academy database, we are unable to change names, mailing addresses or email addresses based on verbal requests. All changes to your personal information should be made by logging into your MyCDR account and editing your profile.

Once you are logged in to your account, click on the “Profile” link on the left side of the blue bar.  If you are making a name change, choose the “Request Name Change” link. Follow the instructions to upload your legal documentation such as marriage license/certificate, divorce decree or court order reflecting your name change.  You will receive an email notification once your request has been processed.  Please allow 72 hours for processing.

RD and DTR Exam Candidates 
Name/address changes made to the CDR database are not automatically forwarded to Pearson VUE. If you are an RD or DTR exam candidate, please be sure to check the box indicating that you will be taking the exam, so that CDR can forward any information changes to Pearson VUE.
