Accessibility and Accommodations
If you are not able to complete the online Learning Plan and Activity Log, it is now mandatory to submit medical documentation for a telephone consultation with CDR Staff. This option will be available for those without a documented medical disability for a fee of $150 for each submission (Learning Plan and Activity Log).
